What You’ll Do
Your responsibilities will include:
- Administering medications and treatments as prescribed.
- Monitoring patient health and reporting any changes to the care team.
- Assisting with daily activities to support patient well-being.
- Providing compassionate, patient-centered care in a home health setting.
- Collaborating with the care team to develop and implement individualized care plans.
What We’re Looking For
To succeed in this role, you’ll need:
- A valid LPN license in good standing.
- At least 1 year of experience in a healthcare setting.
- Strong communication and organizational skills.
- A caring and empathetic nature, with a dedication to patient care.
- The ability to work independently and adapt to the unique needs of each patient.
