Job Title: Physical Therapist (PT) – Long-Term Care / Skilled Nursing Facility (LTC/SNF)

Duration - 13 Weeks

Job Overview

We are seeking an experienced Physical Therapist (PT) for a Long-Term Care/Skilled Nursing Facility setting. This position offers the opportunity to work with residents requiring rehabilitation services focused on improving mobility, function, safety, and overall quality of life.

Schedule

• 8-Hour Day Shifts
• No Weekends
• No On-Call

Requirements

• Active Physical Therapist License (pending license accepted)
• BCLS Certification required
• Minimum 1 year of Skilled Nursing Facility (SNF) and/or Long-Term Care (LTC) experience required
• First-time travelers not accepted
• Reliable transportation required

Responsibilities

• Evaluate residents and develop individualized therapy treatment plans.
• Provide therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence.
• Perform ongoing assessments and adjust treatment plans based on patient progress.
• Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and interdisciplinary teams.
• Educate patients, families, and caregivers regarding rehabilitation goals and home exercise programs.
• Complete timely and accurate clinical documentation.
• Ensure compliance with facility, state, and federal rehabilitation standards.
