Pay Rate: $35.00 - $44.50/hour (Based on Experience)
Shift: Days | 9:00 AM – 5:00 PM
Schedule: Monday – Friday
Position Overview
We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to provide psychosocial support, discharge planning, and care coordination services for patients in an inpatient medicine and surgery setting. The ideal candidate will possess strong assessment, counseling, and case management skills while collaborating with interdisciplinary teams to ensure safe transitions of care and positive patient outcomes.
Required Qualifications
• Master's Degree in Social Work (MSW) from an accredited program.
• Active New York State Licensed Master Social Worker (LMSW) License.
• Primary Source Verification of licensure through NYSED.
• Minimum one (1) year of LMSW experience.
• Experience in a healthcare, hospital, or inpatient setting preferred.
• Strong psychosocial assessment and crisis intervention skills.
• Experience with discharge planning and care coordination.
• Excellent communication, organizational, and advocacy abilities.
• Ability to work effectively with diverse patient populations.
Responsibilities
• Conduct comprehensive psychosocial assessments for inpatient medicine and surgery patients.
• Provide supportive counseling and therapeutic interventions for patients and families.
• Assess emotional, social, financial, and environmental factors affecting recovery.
• Develop and implement individualized discharge plans.
• Coordinate referrals to community agencies, rehabilitation facilities, and home care services.
• Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
• Advocate for patients and families while promoting access to appropriate resources.
• Maintain accurate and timely clinical documentation.
• Participate in multidisciplinary care planning meetings and discharge rounds.
• Ensure compliance with hospital policies, regulatory standards, and professional social work practices.
