We are currently seeking a licensed and compassionate Speech-Language Pathologist (SLP) to join our growing home health care team.
Position Summary:
As a Home Health Speech-Language Pathologist, you will play a vital role in helping patients regain their communication, cognitive-linguistic, and swallowing abilities after illness or injury. You will conduct in-home evaluations and develop individualized treatment plans tailored to each patient's needs. All documentation is completed using the Net Health EMR system to ensure accuracy and regulatory compliance.
Key Responsibilities:
- Perform in-home assessments to evaluate speech, language, voice, cognition, and swallowing
- Develop and implement individualized treatment plans to address patient goals
- Provide direct speech-language therapy services in the home environment
- Educate patients and caregivers on communication techniques and home programs
- Document all evaluations, care plans, and progress notes in Net Health EMR system
- Collaborate with physical therapists, occupational therapists, nurses, and physicians to provide holistic care
- Ensure care is delivered in compliance with home health regulations and agency standards
Qualifications:
- Master’s degree in Speech-Language Pathology from an accredited program
- Current and unrestricted Pennsylvania SLP license
- Certificate of Clinical Competence (CCC-SLP) from ASHA (or CFY candidates with supervision considered)
- CPR certification (or willingness to obtain)
- Valid driver’s license and reliable transportation
- Home health experience preferred but not required – training provided
- Familiarity with EMRs; Net Health experience is a plus
- Strong interpersonal, time management, and communication skills
