Fr333 Yourself Recovery is a residential substance use disorder treatment program dedicated to helping individuals reclaim their lives through compassionate care, evidence-based practices, and holistic recovery support. Our program provides a safe, structured environment where clients can heal and build sustainable recovery.
Position Summary
The Case Manager plays a critical role in supporting clients throughout their recovery journey by coordinating services, assisting with treatment planning, and connecting clients to community resources. The Case Manager works closely with clinical staff to ensure clients receive comprehensive care that addresses substance use, mental health, housing, employment, and other life needs.
Key Responsibilities
- Conduct client assessments and assist in developing individualized treatment and recovery plans
- Coordinate services with therapists, medical providers, probation officers, and outside agencies
- Help clients access resources such as housing, employment, education, and healthcare
- Monitor client progress and provide ongoing support and advocacy
- Maintain accurate client documentation and case notes in accordance with program policies
- Assist with discharge planning and aftercare coordination
- Support clients in developing life skills and relapse prevention strategies
- Participate in multidisciplinary team meetings and collaborate with clinical staff
Qualifications
- Bachelor’s degree in Social Work, Psychology, Human Services, or related field preferred
- Experience working in substance use disorder treatment or behavioral health preferred
- Strong organizational, communication, and problem-solving skills
- Ability to work with diverse populations in a compassionate and professional manner
- Knowledge of community resources and recovery support services
- Background check and TB test required
Required
- CADC, RADT, or other behavioral health certification
- Experience working with justice-involved populations
