Summary

The primary role of the Nurse Delegated Coordinator is to provide in-home support to clients requiring nurse delegated and other medically intensive tasks. Assist clients to achieve their goals in accordance with the residential guidelines. KIM is looking for flexible candidates to work a minimum of two shifts per week and one mandatory on-call weekend once per month.

Salary

$19.00 - $20.00 hourly

Essential Duties and Responsibilities

You must work a minimum of 2 shifts per week.

You must work a mandatory on-call weekend once a month.

Assist clients with daily hygiene as outlined in their individual care plans, including personal care such as brushing teeth, bathing, hair washing, toileting, and more.

Administer correct medications to clients and document the activity accordingly, including nurse delegated tasks.

Communicate with medical offices, pharmacies, and the nurse delegator for medications and appointments.

Assist clients with mobility needs, transferring them from wheelchair to bed, as needed

Monitoring vitals (if applicable) and client behaviors, documenting accordingly.

Transport clients to and from all activities and appointments and/or arrange transportation services as appropriate.

Complete all forms, reports, and data sheets for caseload.

Supervise clients’ daily activities and living conditions to ensure adequate health and safety.

Coordinate individualized programs for clients and document activity thoroughly.

All training required for this position is paid and will be provided by KIM

Certificates, Licenses and Registrations

Nursing Assistant Registered (NAR) License, or higher (required, KIM will assist in obtaining this license upon hire)

Home Care Aide (HCA) License (preferred)

Certified Nursing Assistant (NAC) License (preferred)

Education and Experience

High school graduate or equivalent (required)

Intermediate experience with supporting living, preferably with individuals developmentally or otherwise disabled adults.

Skills and Qualifications

Minimum age of 18 years.

Criminal background check cleared before working alone unsupervised with the clients.

Valid driver’s license or State ID.

Reliable vehicle and Car insurance.

Ability to follow instructions, organize and set priorities.

Effective communication skills, including active listening, writing, speaking and reading comprehension.

In-depth understanding of client care policies and regulations.

Familiarity with symptoms and signs of improvement for a number of medical conditions.
