Summary

Missouri Palliative & Hospice Care is seeking a compassionate and collaborative Extended Care Liaison to provide extended evening and weekend support to patients and families, while serving as a strategic connector between clinical care and sales outreach. This role blends direct patient presence—including vigil care for actively dying patients—with early hospice identification, account coordination, and community-based programming across SNF, AL, and Memory Care settings.


Key Responsibilities

Direct Patient Support

  • Deliver evening/weekend hospice aide services, including personal care, comfort measures, and emotional support.
  • Provide vigil care for actively dying patients, ensuring a peaceful, dignified environment for patients and families.
  • Offer “tuck-in” visits to reinforce safety and comfort heading into overnight hours.
  • Respond to emergent needs with empathy and professionalism, embodying Vigil’s commitment to presence and peace.

Strategic Sales Collaboration

  • Partner with the sales team to identify early hospice-appropriate patients through observation, documentation, and communication.
  • Share timely insights with sales representatives to support compassionate, informed conversations with accounts.
  • Participate in select account huddles or care planning meetings to reinforce Vigil’s clinical presence and build trust with referral sources.

Community Engagement

  • Lead or support group evening activities in strategic accounts (SNF, AL, MC) that promote comfort, connection, and relaxation.
  • Represent Vigil with warmth and professionalism during community events, family nights, or care conferences.
  • Build rapport with staff and residents, positioning Vigil as a proactive, trusted care partner.

Qualifications

  • Certified Nursing Assistant (CNA) or equivalent hospice aide certification required.
  • Minimum 1 year of hospice or geriatric care experience preferred.
  • Strong interpersonal and communication skills; able to collaborate across clinical and sales teams.
  • Evening and weekend availability required.
  • Passion for patient-centered care and community engagement.

Supervisory responsibilities: This position does not involve any direct supervisory responsibilities.

Budgetary/Fiscal Responsibility: Responsible for using resources wisely to support cost-effective operations.

Compliance Responsibility: As an essential function, this position is responsible for complying with the Compliance Program as it applies to the individual job duties, the department, and the company. This position will exercise due diligence to prevent, detect, and report unlawful and/or unethical conduct by fellow co-workers, professional affiliates, and/or agents.

Policy and Procedure Interpretation and Development: Responsible for complying with all company and clinical policies and procedures. Participates in audit process as requested to ensure compliance with policies and procedures. Maintains confidentiality of privileged information and supports the employee’s and patient’s right to privacy.

Work Environment/Schedule/Travel: Routine schedule during standard business hours. Hours worked are based on the assigned patients’ needs. On call shifts as scheduled. Requires frequent travel to and work in patients’ homes within hospice’s service area, and attendance at meetings in office as needed.

Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required care tasks. Frequent driving of own vehicle in various weather conditions. Potential exposure to virus, disease and infection from patients and specimens in working environment. Prolonged or considerable sitting or standing to perform work on a computer, telephone, or other equipment.

Education and experience:

  • A graduate of an accredited high school or equivalent.

Other Job Requirements:

  • Ability to manage multiple tasks, prioritize effectively, and maintain orderly records and systems.
  • Strong verbal and written communication skills are essential for interacting with colleagues, clients, and stakeholders.
  • Must possess digital literacy, including familiarity with various computer applications and internet-based tools. Proficiency in using standard Microsoft/office software and applications (e.g., email, word processing, spreadsheets) is essential along with basic data entry skills, as well as the ability to quickly learn and adapt to new software and systems used in our office.
  • Flexibility to adjust to changing priorities, tasks, and environments.

Requirements:

Qualifications & Skills

  • Certification: Current Home Health Aide (HHA) certification in good standing (state-specific).
  • Experience: Previous hospice, palliative, or elder care experience strongly preferred.
  • Engagement Skills: Ability to lead group activities and foster connection.

  • Dependability: Reliable transportation and flexibility to travel between locations.
  • Availability: Evening hours required.
