INTRODUCTION
Under the supervision of the Nurse Manager-Family Planning, the Family Planning Care Associate provides family planning and related health services education, resources, counseling, and care coordination to Health Center patients and individuals in the community in order to assist them to meet their sexual and reproductive health goals. The Family Planning Care Associate is responsible for providing sexual and reproductive health education resources and training to health center staff, in order to promote high quality care and patient engagement.
MAJOR DUTIES/ESSENTIAL FUNCTIONS
Essential and other important responsibilities and duties may include, but are not limited to the following:
- Works at various sites providing one-on-one and group family planning education both independently and in coordination with medical providers.
- Provides outreach and delivers culturally appropriate family planning education.
- Collaborates other departments to orient new hires on Title X program and Family Planning services at Unity.
- Supports the provision of technical training and assistance to clinical staff as it relates to Family Planning and related health services.
- Organizes and maintains family planning and related services education materials at assigned health centers.
- Collaborates with other internal programs with regard to community and client-focused events, such as health fairs and educational classes to increase number of family planning clients seen at Unity Health Centers.
- Maintains assigned projects in order provide care coordination for patients in need of family planning and related health services. (e.g., call patients to remind of appointments and reschedule missed appointments).
- Accurately documents all patient encounters in electronic health record within established time frame.
- Participates in quality assurance and improvement with regards to family planning stock management and education materials.
- Assists in identification and implementation of new communication strategies for FP patient education (e.g., Unity’s patient portal, social media, etc.)
- Under the direction of the Family Planning leadership, collects and compiles data for quality improvement activities and other projects as assigned; maintains data in an organized manner.
- Participates in family planning staff meetings as well as monthly staff meetings at assigned clinic sites as directed.
- Performs other duties as assigned.
MINIMUM QUALIFICATIONS
- Associate’s degree. Concentration in public health or community health a plus. Bachelor’s degree is preferred.
- Graduate of accredited Medical Assistant program.
- Successful completion of family planning health educator training must be completed within 60 days of hire.
- Active CPR license.
- Six months of community health experience. One year of clinical and public health experience in primary care or similar setting preferred.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
- Ability to work independently with youth and adults in a variety of settings.
- Ability to demonstrate openness, flexibility and respect when providing information about reproductive health issues.
- Willingness to continue education/competency training in family planning and sexual, reproductive health services.
- Ability to communicate effectively, verbally, and in writing.
- Able to learn new skills, procedures, and information as the standards of care change.
- Ability to commute to various clinics, sometimes more than one in the same work day.
- Willingness to articulate Unity’s mission through their work.
- Demonstrated proficiency with business software (e.g. Microsoft Office Suite preferred). Experience in the use of computerized filing/spreadsheets systems preferred.
- Experience with Family Planning populations preferred.
- Bilingual: English/Spanish preferred.
SUPERVISORY CONTROLS
The position reports to the Nurse Manager-Family Planning.
GUIDELINES
The position follows OSHA standards of safety and universal precautions, as well as Unity policies and procedures.
PERSONAL CONTACTS
The position requires contact with staff at all levels throughout the organization. There are also external organization relationships that may be a part of the work of this individual.
PHYSICAL EFFORT AND WORK ENVIRONMENT

RISKS
The position involves everyday risk and discomforts, which require normal safety precautions typical of such places as offices, meetings, training rooms, and other UHC health Care Sites. The work area is adequately lit, heated, and ventilated. All medical services shall be provided according to medically accepted community standards of care. The employee shall provide evidence of a recent (within the past twelve (12) months) health assessment that includes a PPD and/or chest x-ray results.
