JOB DESCRIPTION IN HOME SERVICE COORDINATOR
Position Title: In-Home Service Coordinator
Reports To: Registered Nurse & Director
Qualifications: High school education, computer skills, good organizational skills, and experience
working with the elderly preferred.
Responsibilities:
· Maintain a register of qualified In-Home Service Workers
· Schedule In-Home Service Workers
· Create and prepare timesheets, logs, and travel forms for the In-Home Service Workers
· Collect and review completed timesheets, logs, and travel forms and reconcile these records with the In-Home Service Workers’ schedules
· Maintain and utilize the Relias training software for In-Home Service Workers
· Complete continuing certification information as required for the Medicaid Waiver Program
· Complete employment documents and fingerprint records for new employees of the Barbour County Senior Center and provide orientation for the In-Home Service Programs
· Maintain training as an Alzheimer’s trainer and train employees as needed
· Document correspondence with clients
· Maintain In-Home Service clients’ files to insure required documentation is in place
· Assemble and maintain In-Home Service Workers’ personnel files to insure that required training and other documentation is in place.
· Randomly check with clients to determine if In-Home Service Workers are reporting to work on the scheduled day and time and completing their work as assigned.
· Maintain visits and records for the Family Alzheimer’s In-Home Respite, the Title III B, Title IIIB Homemaker and the Title IIIE In-Home Respite Programs
· Bill and maintain billing records for Lighthouse, FAIR, IIIB and IIIE Programs.
· Assist the registered nurse as needed
· Read, comprehend, and update (as needed) the In-Home Service Program manual. Answer the telephone if requested to do so.
