Job Description
We are seeking for a compassionate, motivated, and clinically skilled PMHNP to support our growing behavioral health services in nursing homes throughout Niagara County, NY. We are dedicated to delivering exceptional psychiatric care to older adults, and we’re looking for a practitioner who shares our passion for improving mental health outcomes in the geriatric population.

RequirementsPrimary Responsibilities

  • Conduct in-depth psychiatric assessments and evaluations for residents within skilled nursing and long-term care facilities.

  • Identify mental health conditions and create individualized, evidence-based treatment plans tailored to each patient.

  • Manage and adjust psychiatric medications while monitoring patient progress and response.

  • Work collaboratively with an interdisciplinary care team—including psychologists, physicians, nurses, and social workers—to ensure coordinated and comprehensive treatment.

  • Provide therapeutic support, including counseling and psychotherapy, as appropriate.

  • Educate residents and their families about diagnoses, medication use, treatment goals, and strategies for maintaining mental wellness.

  • Maintain accurate, timely, and compliant documentation within the electronic health record system.Participate in case discussions, interdisciplinary meetings, and quality improvement projects aimed at enhancing patient care



  • Qualifications


  • Active New York State Nurse Practitioner license with PMHNP certification.

  • Ability to work independently while effectively managing a varied caseload.

  • Strong clinical assessment, diagnostic, and medication management skills.

  • Excellent communication and interpersonal abilities with the capacity to build trust and rapport with patients and staff.

  • Proficiency in EHR systems and basic computer applications.

  • A compassionate, patient-centered approach to psychiatric care.
