Home Health Care Experience: Required (Minimum 2 years)
Certifications: Updated BLS/CPR and Negative PPD Required

Key Responsibilities:
- Provide patient care under the direction of an RN
- Identify and assess physical, psycho-social, and environmental needs of patients
- Verify and follow the Plan of Care prior to each visit
- Deliver care per physician’s orders and document all interventions accurately
- Incorporate short- and long-term goals into care plans
- Conduct ongoing assessments during each visit and document findings
- Educate patients and caregivers on disease processes, treatment regimens, and medication administration
- Recommend additional disciplines as needed to support patient outcomes

Compensation: Starting from $60,000 annually, then per-visit rate thereafter
