CCM Program Director (5 years' minimum experience required)
Job Description
Saisystems Health is seeking a CCM Program Director to join our team! This individual will play a pivotal role in leading and expanding Saisystems Health’s care management services. This individual will drive revenue growth, enhance customer service levels, develop and oversee clinical programs, and ensure high customer satisfaction. This individual will work closely with senior management to align the operational strategy with the company's broader goals, making key decisions that affect the company's direction in care management.
**Only Candidates with 5 years of proven experience with Chronic Care Management will be considered**
Job Responsibilities:
- Strategic Leadership: Develop and implement operational strategies that enhance profitability and efficiency across the company’s care programs.
- Revenue Management: Drive the profitability of care management services, including budgeting, forecasting, and resource allocation to ensure financial targets are met.
- Program Development: Oversee the design and enhancement of clinical programs to meet the evolving needs of patients and to comply with regulatory standards.
- Customer Service Oversight: Ensure exceptional service levels are maintained across all customer touchpoints. Develop strategies to enhance patient engagement and satisfaction.
- Team Management: Lead, mentor, and develop a high-performing team of managers and healthcare professionals dedicated to care management.
- Performance Metrics: Monitor and report on operational performance metrics, making data-driven decisions to optimize processes and outcomes.
- Stakeholder Engagement: Partners with billing, clinical, credentialing and vendor teams to align workflows, with care management and financial goals.
- Champions adherence to CMS guidelines, HIPAA regulations, and billing requirements across related billing codes.
Education Necessary for Position:
- Bachelor's degree in Healthcare Management, Administration, or related field
Experience & Skills Necessary for Position:
- Minimum of 5 years’ experience in a senior management role in a healthcare setting (LTC, SNF), with a strong emphasis on care management, including but not limited to Care Management and Behavioral Health Integration.
- Extensive experience in managing clinical programs, customer relations, and team leadership.
- Demonstrable track record of improving service delivery and achieving financial success within a healthcare operation.
- Strong leadership skills with the ability to drive on and offshore team performance and foster a culture of excellence and accountability.
- Excellent analytical and problem-solving abilities, with a keen focus on operational efficiency and strategic development.
- Proven ability to manage and oversee budgetary operations in a healthcare setting.
- Deep understanding of care management and the healthcare industry, including regulatory and market dynamics.
- Ability to innovate and integrate technology and business solutions into care practices.
Work Environment:
This position will be a hybrid, candidates encouraged from MA, NY, RI, NJ and CT. Every two weeks, employee will be required to come to Shelton, CT office for 3 (three) days.
