Full-Time School-Based Case Manager Position in Keene, New Hampshire!

Were looking for a compassionate Case Manager to join our team in Keene, NH for the upcoming school year. If you have a background in case management and are ready to make a difference in a school setting, this is the perfect opportunity for you!

Responsibilities:

• Coordinate and manage services for students with special needs.

• Collaborate with teachers, parents, and specialists to implement Individualized Education Plans (IEPs).

• Monitor student progress and adjust plans as needed to ensure success.

• Provide resources and support to families to enhance students' educational experience.

Qualifications:

• Active State Licensure or Certification in New Hampshire.

• Previous school-based experience in case management or related roles.

• Bachelors or Masters degree in Social Work, Psychology, or a related field.

Position Details:

• Classroom: K-12

• Hours per Week: Full-Time (35-40 hours)

• Weekly Pay: Competitive pay based on experience

Contract Duration:

• Current school year with the option to extend for summer school and the 2024-2025 school year.
