Job Description1st Call Home Healthcare is hiring a full time RN/LPN Homecare Nurse Coordinator to cover our West Michigan Territory. Must love to drive coverage area includes Grand Rapids, Battle Creek and Kalamazoo areas. $$$ 5,000 Sign on Bonus $$$
The Nurse Coordinator assumes responsibility and accountability for the delivery of industry recognized home care nursing and health related services to the agency’s patients
Duties Include:
● Perform start of care visits and establish care plans. Monthly follow up visits.
● Perform Supervisory Visits to client and train and supervise caregivers
● Collaborate with interdisciplinary groups and coordinate care
● Handle emergency phone lines on off hours
● Participate in community marketing and customer relation activities
● Perform nurse shift duties
● On occasion, assist Clinical Manager with Quality Assurance chart audits
What we offer:
Salary commensurate with experience beginning at $75,000 annually with $5,000 sign on bonus to be paid in instalments
