Job Overview

Comfort Creek Nursing & Rehab Center is seeking a compassionate and motivated Certified Occupational Therapy Assistant (COTA) to join our growing in-house rehabilitation team. We are hiring for Full-Time, Part-Time, and PRN (Per Diem) positions with flexible scheduling options designed to support work-life balance.

Working under the direction and supervision of a licensed Occupational Therapist, the Certified Occupational Therapy Assistant will provide skilled therapy services that help residents regain independence in activities of daily living (ADLs), improve functional performance, enhance safety, and maximize quality of life following illness, injury, surgery, or hospitalization.

Essential Duties and Responsibilities

  • Implement individualized occupational therapy treatment plans developed by the supervising Occupational Therapist.

  • Provide skilled occupational therapy interventions to improve residents' ability to perform activities of daily living (ADLs), instrumental activities of daily living (IADLs), upper extremity function, coordination, balance, strength, cognition, and fine motor skills.

  • Assist residents with self-care training, adaptive techniques, energy conservation, home management skills, and functional mobility as appropriate.

  • Monitor resident progress and communicate changes in condition or treatment response to the supervising Occupational Therapist.

  • Accurately document treatment sessions, resident responses, progress notes, and other required documentation in accordance with Medicare, Medicaid, state, federal, and facility guidelines.

  • Educate residents, families, and caregivers on therapeutic exercises, adaptive equipment, home safety, fall prevention, and strategies to promote independence.

  • Assist with splinting programs, positioning, adaptive equipment recommendations, and environmental modifications as directed by the Occupational Therapist.

  • Collaborate with Occupational Therapists, Physical Therapists, Speech-Language Pathologists, nursing staff, physicians, and other interdisciplinary team members to coordinate resident care.

  • Participate in care conferences and interdisciplinary team meetings as appropriate.

  • Maintain compliance with all regulatory, professional, and facility standards.

  • Foster a positive, resident-centered rehabilitation environment focused on quality outcomes and clinical excellence.

QualificationsRequired

  • Graduate of an accredited Occupational Therapy Assistant program.

  • Current Georgia Certified Occupational Therapy Assistant (COTA) license in good standing.

  • Current CPR certification or willingness to obtain upon hire.

  • Strong communication, organizational, and interpersonal skills.

  • Ability to work effectively within a collaborative interdisciplinary rehabilitation team.

Preferred

  • Experience in skilled nursing, subacute rehabilitation, long-term care, inpatient rehabilitation, or post-acute care.

  • Familiarity with Medicare documentation requirements.

  • Experience treating geriatric and medically complex patient populations.

  • Knowledge of evidence-based occupational therapy interventions, adaptive equipment, fall prevention, cognitive rehabilitation, and functional training.
