As a member of the Eleos treatment team, the Discharge Planner completes individualized assessments, treatment planning, case management, and discharge planning for mental health clients during their inpatient stay. Discharge Planners engage with clients on the Crisis Stabilization Units (CSUs) on an ongoing basis to identify and confirm appropriate outpatient placement and sheltering arrangements upon their release from the facility. Attention to detail and thorough documentation of all efforts made are essential to this role.
Shift: Monday through Friday, 8am - 4pm. Occasional overtime, weekends, or holidays may be required.
Education: Bachelor’s or master's degree in Social Work or Human Services field. (Additional $1.00 per hour compensation for master's.)
Experience: One (1) year of experience as mental health social worker (preferred) or one (1) year of other relevant work experience.
Physical Requirements: Must be able to sit, twist, bend, and stand. Must be able to communicate both orally and in writing.
Special Requirements: Exposure to consumers; treatment program conditions, high turnover in consumer census with specific paperwork on each consumer.
