HomeCentris Personal Care Division is seeing a Community Health Worker to help us grow in the Philadelphia, PA area!

The Community Health Worker is primarily responsible for establishing and maintaining community relationships with local senior organizations, referral sources and prospective home care clients. The Community Health Worker is the face of HomeCentris in each local community. The position will provide outreach and home care education to local community organization, senior living facilities, and to potential clients so that senior may remain in the community as they age.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

• Responsible for driving client growth and developing new referral relationships
• Develop new referral sources by planning and organizing schedule to call on potential referral outlets and other sources of prospective clients.
• Meets with prospective clients in their homes to discuss agency’s services, payment options, onboarding process.
• Assists the agency’s Community Health Worker in processing and on-boarding new clients.
• Communicates with and supports existing clients as needed in conjunction with PHC’s Client Relationship Managers.
• Keeps management informed by updating CRM systems, submitting sales activity and results reports, such as daily call reports, weekly work plans, and monthly and annual territory analyses.
• Develops “out of the box” sales strategies/models and evaluates their effectiveness.
• Assist with developing recruitment strategies to attract candidates.
• Collaborates closely with Recruiting and Human Resources to ensure candidates complete onboarding process.
• Other duties as assigned.

Requirements:

• Proven relationship with senior apartment building, assisted living facilities, senior center, etc.
• Bilingual candidates encouraged to apply.
• Prior experience with Community Outreach activities.
• Sales and marketing experience.
• Customer serviced focused.

Pay: $45,000.00 - $53,000.00 per year
