Job DescriptionJoin Our Team as a Physical Therapist

We are looking for a committed Physical Therapist to become a part of our Healthcare & Medical Services team, delivering exceptional, patient-focused rehabilitation care. In this position, you will assess, plan, and implement evidence-based physical therapy interventions aimed at helping patients restore function, enhance mobility, alleviate pain, and improve their overall quality of life across various care environments.

As a Physical Therapist, you will collaborate closely with a multidisciplinary team, which includes nurses, physicians, occupational and speech therapists, and social workers. Together, you will create personalized treatment plans, establish functional objectives, and continuously evaluate patient progress to ensure safe and effective outcomes.

Key Responsibilities

  • Conduct thorough physical therapy evaluations, assessing strength, range of motion, balance, gait, and functional mobility.
  • Document findings meticulously in the electronic medical record.
  • Design and execute tailored treatment programs that may include therapeutic exercises, neuromuscular re-education, balance and gait training, manual therapy, and education for patients and their families.
  • Instruct patients and caregivers on home exercise routines, safe mobility practices, fall prevention strategies, and the correct use of assistive devices like walkers, canes, and wheelchairs.
  • Work collaboratively with the care team to coordinate services, modify care plans based on clinical assessments, and engage in care conferences for progress and discharge planning.
  • Ensure timely and accurate documentation that complies with regulatory, payer, and organizational standards while adhering to state practice regulations and professional guidelines.
  • Support quality improvement initiatives, contribute to outcome tracking, and maintain infection prevention and safety protocols.
