Job DescriptionThe Clinical Coordinator for the Recollections Memory Care Unit is responsible for overseeing all clinical operations within the dementia care neighborhood. Working collaboratively with the Recollections Co-Coordinator, this nurse ensures the highest quality of care for residents by managing clinical staff, monitoring resident health, coordinating medical services, and maintaining compliance with professional standards and Department of Health regulations. The ideal candidate is empathetic, compassionate, patient, and committed to supporting residents with cognitive impairment in a safe and nurturing environment.

Responsibilities
RESPONSIBILITIES:
- Partner with the Recollections Co-Coordinator to effectively operate and support the Memory Care Unit.
- Provide clinical leadership and supervision to resident aides, including delegating assignments in accordance with each resident’s individualized Plan of Care.
- Monitor residents’ medical conditions, promptly identify changes in condition, and communicate with physicians, families, and appropriate healthcare providers.
- Oversee the medical needs of all residents within the unit to ensure timely and appropriate care.
- Administer medications and treatments as ordered by physicians.
- Maintain medication carts to ensure organization, proper storage, and compliant disposal of medications.
- Ensure accurate, timely, and compliant documentation in accordance with professional nursing standards and Department of Health guidelines.
- Participate in semi-annual care conferences with residents’ families and attend additional meetings as needed.
- Assess the environment regularly to promote safety, functionality, and infection control best practices.
- Support a culture of dignity, respect, and person-centered care for residents living with dementia.
- Serve as a clinical resource for staff and assist with problem-solving related to resident care.
QUALIFICATIONS:
- Active and unrestricted Licensed Practical Nurse (LPN) license in good standing.
- Previous experience in memory care, geriatrics, long-term care, or assisted living strongly preferred.
- Demonstrated leadership or supervisory experience is preferred.
- Strong clinical assessment and critical thinking skills.
- Knowledge of infection control practices and regulatory compliance.
- Excellent communication and interpersonal skills with the ability to collaborate effectively with families, physicians, and interdisciplinary teams.
- Compassionate approach with patience and understanding when caring for individuals with dementia.
Requirements
Must be at least 21 years of age
LPN License in good standing
