Location: Grand Junction, Colorado
Pay: $48–$55/hour DOE + $5,000 Sign-On Bonus
Schedule: Flexible, autonomous daytime schedule

What You'll Do

Deliver personalized OT services in patients' homes

Develop functional, individualized treatment plans

Complete home safety evaluations and recommend adaptive equipment

Collaborate with an interdisciplinary care team (nursing, PT, caregivers, families)

Document care efficiently with full EMR training and support

Travel locally within Mesa County (mileage and drive time reimbursed)
What You Bring
-
Active Colorado Occupational Therapist license (required)

Home health experience preferred, not required

Strong clinical judgment and comfort working independently

Excellent communication and time-management skills

Reliable vehicle, valid driver's license, and auto insurance

A genuine passion for helping people maintain independence and dignity
