Qualifications:
Bachelor’s degree in a related field: Human Services and Social Work
Travel Requirements: 50-75% (Regional/Local)
Schedule: 40 hrs per week, Occasional Weekend / Evening hours
Duties & Responsibilities:
The Prevention Specialist II is to assist in ensuring effective implementation of prevention services within serving counties of HopeHealth, Inc., according to community and agency needs.
· Knowledgeable/oriented of grant logistics
· Perform data entry, record keeping, DHEC surveillance reporting, Provide Enterprise and
Documentation and quality assurance for testing inventory
· Conducts HIV/Hepatitis C, substance abuse screenings and facilitates DEBI interventions
· Provide condom distribution (monthly/quarterly) in accordance to community needs
· Make appropriate referrals for clients to obtain adequate and relevant services as identified in
their assessments
· Assist/Participate in community events to include: community/church health fairs, DAODAS sites,
housing authorities, detention centers, college campuses, health facilities, etc.
· Provide/Assist with services to expedite linkage to care for clients new to the agency [i.e.
obtaining proof of eligibility, proof of HIV labwork, identification card, transportation to obtain
income documentation
· HIV testing of partners, family members and appointment companions of clients returning to
care.
· Documents and ensures confidentiality as well as data security while on, community and during
transportation to/from events, etc.
· Adhere to all quality assurance procedures, policies, and guidelines within grant, agency and
health department
· Participates and adhere in team activities, meetings, trainings, events etc. This includes quality
improvement meetings, supervisory meetings, and agency-specific meetings as indicated
· Travel to trainings, partner meetings and testing locations
· Attends DHEC and CDC-required trainings, webinars and conferences
· Performs other job duties as requested by Supervisor or Site Manager
Skills & Knowlege Required:
Excellent communication and organization skills, Knowledge of HIV/Hepatitis C
and STD’s, and knowledge of Microsoft XP Tools
