Job Summary: Provides a broad range of paraprofessional social services to individuals and their families. May provide supportive counseling to individuals and their families and/or serve as liaison for social services. May monitor operations of training or personal support residence. May functions as a supervisor or lead worker for other paraprofessionals.

- Ensures that the charts assigned to their caseload are in compliance according to agency, state and federal guidelines.
- Works in collaboration with staff assigned to worksite to ensure all of the necessary documentation is completed in a timely and accurate fashion.
- Prepares and documents pertinent reports for/to supervisors, colleagues and other appropriate individuals in a timely manner.
- Participate and provide input in staff departmental meetings, treatment team meetings, annual ISPs, addendums, and other pertinent meetings.
- Participate in intake process and collects information from individuals, families, direct care staff, community agencies and physicians determine appropriate goals and services.
- Assist individuals with community awareness, integrating in community activities and leisure activities, and provide training in socialization with in the community.
- Schedules follow-up psychiatric and medical appointments in a timely manner to ensure that necessary services are provided.
- Serves a liaison between individuals and family members, significant others, physicians, organizational components, etc., to facilitate and ensure communication, effective relationships, linkage and continuity of services.
- Monitors individual’s progress towards treatment goals.
- Develops community resources and contacts to facilitate delivery of supported employment services.
- Attend seminars, conferences, and job related trainings.
- Conducts the following services, supports, care and treatment, including but not limited to:
A. Overseeing the services, supports, care and treatment provided to individuals;
B. Supervising the formulation of the individual service plan or individual recovery plan;
C. Conducting diagnostic, behavioral, functional and educational assessments;
D. Designing and writing behavior support plans;
E. Implementing assessment, care and treatment activities as defined in professional practice acts; and
F. Supervising high intensity services such as screening or evaluation, assessment, partial hospitalization, and ambulatory or residential crisis services.
Required Knowledge & Skills:
- Knowledge of state, local and federal guidelines in order to modify existing programs and the development of new programs in response to changes in the guidelines.

Required Education & Experience: Bachelor’s degree in a social service field and one year of full-time (or equivalent part-time) experience in a social services delivery Or a master’s degree in a social service field. Must possess a valid GA driver’s license.

Work Environment:
This job operates in a variable business settings with trips into the community. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This role provides basic employment support which requires employee to perform in loud/quiet environments, outdoors/indoors, etc. Some medium travel between Gateway sites and in the community is required.
Physical Demands:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
Employee is frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Employee must frequently lift and/or move up to 15 pounds. Must have the ability to sit for long periods of time at a computer. Employee frequently uses fine hand/eye coordination, hearing and visual acuity. Lighting and temperature are adequate, and there are not hazardous or unpleasant condition caused by noise, dust, etc. Employee must be able to travel between Gateway sites. Any employee may be required to stay at or return to work during agency / program incidents and/or emergencies to perform duties specific to this position or perform other duties as requested in an assigned response position. This may require working a non-traditional work schedule or working outside normal assigned duties during the incident and / or emergency.
