Job Description
Job DescriptionSalary: $25 - $26 per hour DOE
RESPONSIBILITIES:
- Complete Initial assessments of service needs and develop a comprehensive, individualized care plan and collect and update eligibility documents for clients, annually.
- Schedule appropriate follow-up appointments with clients to monitor care plan progression and provide linkages to health and support services to maintain continuity of care.
- Provide ongoing assessment of clients needs and personal support system through re-evaluation of the care plan at least every 6 months with adaptations as necessary. Discharge as appropriate.
- Participate in regularly scheduled case conferences and multidisciplinary team meetings, and present as assigned.
- Maintain current documentation in EMR of all client and collateral contacts
- Provide treatment adherence support through appointment reminders, referral management, and patient assistance programs.
- Keep abreast of new information related to the effectiveness of case management for persons living with HIV through in-service and external trainings, supervision, and the RW Subcommittee
meetings.
- Aid with the completion of medical documents to ensure benefits, support services, medical equipment, or other needs are met.
- Any other duties as assigned.
Office Hours: Monday through Friday, 8:30am 5:30pm
Telework Eligibility: Up to 50% following successful completion of initial employment period
Qualifications: Associates/Bachelors degree in health or human services related field preferred. High School diploma or GED required. A minimum of 2 years of experience working with persons living with HIV or at high risk of HIV acquisition preferred.
