Description
American Traveler is hiring an experienced RN for a Care Coordinator/Case Management position at a Level II Teaching Hospital, requiring at least 1 year of experience and an active PA or compact RN license.
Details
- Works in the Care Coordinator/Social Services unit of a 261-bed acute care, Level II teaching hospital
- Patient caseload ratio of 15–20 patients
- Day shift schedule, 5x8-hour shifts (8:00–16:30)
- No weekend requirement, though weekend shifts are available for those who wish to work them
- No on-call requirements
- Charting is performed using Epic
Requirements
- Active PA RN license or compact RN license currently in hand (pending license not accepted)
- Current BLS certification required
- Minimum 1 year of RN experience required
- First-time travelers will not be considered for this position
Additional Information
- Coordinate patient care and social services needs within an acute care hospital setting
- Wine-colored scrubs are the standard dress code, though wearing scrubs is not mandatory
- Candidates who have previously worked at any Main Line Health location should disclose the location(s) and dates of employment
- Time off requests must be submitted at the time of initial consideration
- 2 professional references from a supervisor within the past 2 years are required
- Full 7-year work history with all gaps explained is required
