Job Title: Nurse Case Manager II
Location: Near to Downers Grove, IL (Limited Field/Office Visits)
Duration: 06 Months
Pay rate: $38.00-$41.00 on W2
Role:
Care Manager RN supporting care management and care coordination across medical and behavioral health. Focus on assessments, care planning, provider collaboration, and member engagement.
Key Highlights:
- Telephonic care management (90%)
- Occasional member visits (within 1 hour, if requested)
- Office visit once per quarter (Downers Grove, IL or Teams if >2 hrs away)
- Patient-facing, hybrid role
- HIPAA-compliant home workspace required
Requirements:
- Active, unrestricted IL RN license
- 3–5 yrs clinical experience (hospital, ambulatory, outpatient)
- Case management / managed care preferred
- Strong clinical judgment & communication skills
- Spanish preferred (not required)
