Nurse Practitioner (NP) - Long-Term Care
Location: Pittsburgh, PA
Schedule: Monday-Friday, 8am-5pm (40 hours/week)
Overview
A reputable healthcare organization is seeking an experienced Nurse Practitioner to provide primary and preventative care services to residents in skilled nursing facilities. This role is ideal for a clinician passionate about caring for adults and seniors with complex and chronic conditions in a long-term care environment.
Key Responsibilities

Provide primary and preventative care to long-term care patients residing in skilled nursing facilities

Manage residents experiencing acute changes in condition

Conduct advanced care planning and chronic care management

Prescribe appropriate medications, including controlled substances, in accordance with Pennsylvania law
-
Order, review, and follow up on treatments, diagnostic tests, and lab results
-
Communicate regularly with nursing home staff regarding changes in patient condition and care plans
-
Collaborate closely with the primary care physician to ensure comprehensive, patient-centered care
Must-Have Qualifications (3-5)
-
Current, unrestricted Nurse Practitioner license in the state of Pennsylvania

National Board Certification
-
Active DEA licensure
-
Clinical background in adult, family, or geriatric specialties

Experience working in a long-term care or skilled nursing facility setting

Proficiency with EMR systems

Ability to manage patients with advanced illness and multiple chronic conditions
Nice-to-Have Skills

Strong communication, empathy, and relationship-building skills

Ability to work both independently and collaboratively with facility staff
