Care Coordinator Compensation
- Competitive pay rate of $27.17 per hour
Care Coordinator Highlights
- Monday–Friday, 8:00am–5:00pm (flexibility required)
- Remote/work from home position (must reside in Hardin, Allen, Darke, Logan, or Shelby County, OH)
- Up to 50% travel within Western Ohio (member home visits and community locations)
- Occasional travel (1–2 times per year) to Franklin County for meetings/trainings
- Mileage reimbursed per company policy
- Training conducted remotely
Care Coordinator Responsibilities
- Coordinate case management activities for children and youth enrolled in the OhioRISE program
- Conduct telephonic and face-to-face comprehensive member assessments
- Develop, implement, and monitor individualized care plans
- Collaborate with Child & Family Teams and multidisciplinary providers
- Identify high-risk factors and service gaps impacting member outcomes
- Utilize motivational interviewing to increase member engagement
- Connect members to community-based services and resources
- Identify and escalate quality-of-care concerns
- Ensure documentation complies with regulatory and accreditation standards
Care Coordinator Requirements
- Bachelor’s degree or non-licensed Master’s degree required (Social Work, Psychology,
- Counseling, Human Services, Public Health, or related field)
- 2+ years of experience in behavioral health, social services, or human services
- 2+ years of experience working with children or adolescents (mental health, child welfare, developmental disabilities, juvenile justice, or community-based services)
- Microsoft Office proficiency (Outlook, Word, Excel, SharePoint)
- Verifiable High School Diploma or GED required
- Reliable transportation required
- Willing and able to travel up to 50% locally
- Strong communication and documentation skills
Preferred Qualifications
- Case management and discharge planning experience
- Managed care experience
- Medicaid experience
- Knowledge of Ohio community systems and resources
