Care Coordinator Compensation

  • Competitive pay rate of $27.17 per hour


Care Coordinator Highlights

  • Monday–Friday, 8:00am–5:00pm (flexibility required)
  • Remote/work from home position (must reside in Hardin, Allen, Darke, Logan, or Shelby County, OH)
  • Up to 50% travel within Western Ohio (member home visits and community locations)
  • Occasional travel (1–2 times per year) to Franklin County for meetings/trainings
  • Mileage reimbursed per company policy
  • Training conducted remotely


Care Coordinator Responsibilities

  • Coordinate case management activities for children and youth enrolled in the OhioRISE program
  • Conduct telephonic and face-to-face comprehensive member assessments
  • Develop, implement, and monitor individualized care plans
  • Collaborate with Child & Family Teams and multidisciplinary providers
  • Identify high-risk factors and service gaps impacting member outcomes
  • Utilize motivational interviewing to increase member engagement
  • Connect members to community-based services and resources
  • Identify and escalate quality-of-care concerns
  • Ensure documentation complies with regulatory and accreditation standards


Care Coordinator Requirements

  • Bachelor’s degree or non-licensed Master’s degree required (Social Work, Psychology,
  • Counseling, Human Services, Public Health, or related field)
  • 2+ years of experience in behavioral health, social services, or human services
  • 2+ years of experience working with children or adolescents (mental health, child welfare, developmental disabilities, juvenile justice, or community-based services)
  • Microsoft Office proficiency (Outlook, Word, Excel, SharePoint)
  • Verifiable High School Diploma or GED required
  • Reliable transportation required
  • Willing and able to travel up to 50% locally
  • Strong communication and documentation skills


Preferred Qualifications

  • Case management and discharge planning experience
  • Managed care experience
  • Medicaid experience
  • Knowledge of Ohio community systems and resources
