Under the guidance of our primary care operations team, the Primary Care Patient Care LPN collaborates closely with our Primary Care Providers to deliver safe, timely, effective, and patient-centered care. This role emphasizes a systematic approach to ensure equitable and efficient healthcare. The Primary Care Patient Care LPN will work alongside patients, providers, and other clinical team members to enhance quality outcomes for every individual.

Position Overview:

• Experience: A minimum of 6 months in a clinical healthcare setting, ideally within a primary care environment.
• Education: Completion of an accredited practical nursing program is required.
• Licensure: Must hold a valid LPN (Licensed Practical Nurse) license in the state of practice.
• Additional Skills: Ability to function effectively as part of a team and collaborate with all care team members. Strong written and verbal communication skills are essential. A customer service mindset, along with excellent organizational and interpersonal abilities, is necessary. Candidates should demonstrate flexibility, a positive attitude, attention to detail, and the capability to work independently within their scope of practice with minimal supervision.
