Job Summary:

This position will ensure that the health care needs of all clients are met.

The RN Candidate will ensure that staff is trained in assisted living procedures and policies which are current and up to date. The Registered Nurse, RN will also ensure that Cornerstone Management assigned communities are in compliance with current Assisted Living License regulations. Working cooperatively with the DON to supervise the delivery of services provided to the clients and supervises staff.

This LPN Candidate will ensure that the health and safety of all residents are met. The LPN will assist the RN with planning, coordinating, and managing the assisted living services and act within the Assisted living community regulations. The LPN is responsible for the overall direction, coordination, and monitoring of assisted living service delivery under the direction of the RN.

Key Responsibilities:

  • General Responsibilities
  • Oversee day-to-day clinical services and resident care
  • Ensure compliance with state/federal assisted living regulations and lead during surveys
  • Conduct resident assessments, update care plans, and manage service agreements
  • Supervise unlicensed staff, conduct training and performance evaluations
  • Manage medication administration and documentation; follow up on physician orders
  • Serve as on-call clinical support for staff and resident emergencies
  • Maintain accurate records, support infection control, and ensure resident safety
  • Communicate with families, caseworkers, and healthcare providers
  • Collaborate with the Director of Nursing (DON) and leadership team on care strategy and operations.

  • Resident Care & Clinical Support
    • Provide nursing support in accordance with Assisted Living regulations and community standards.
    • Monitor resident health, update care plans, and recommend alternative placement when necessary.
    • Assist with medication management: fill pillboxes, monitor renewals, and coordinate with pharmacies and physicians.
    • Communicate and document changes in resident condition clearly and promptly.

  • Compliance & Documentation
    • Maintain accurate and up-to-date resident records and nursing notes.
    • Support regulatory compliance efforts; assist in preparation for state licensing surveys.
    • Ensure privacy and confidentiality of resident information and living environments.

  • Team Collaboration & Communication
    • Work closely with RNs and care staff to deliver high-quality, resident-centered care.
    • Share key updates through communication tools (e.g., staff communication book).
    • Participate in team meetings and contribute to ongoing service improvements.
    • Maintain open, respectful communication with residents, families, and team members.
    • Carry a cell phone and be available for emergencies or staffing updates.
    • Respond to resident concerns and complaints with professionalism and compassion.
    • Support events, trainings, and family functions as needed.
    • Uphold safety standards and report incidents or hazards promptly.

Qualifications:

  • Current RN or LPN license (unrestricted) in state of employment
  • Minimum 3 years of nursing experience, preferably in assisted living or long-term care
  • Valid driver’s license
  • Strong organizational, leadership, and communication skills
  • Comfortable with PRN scheduling involving weekends.
  • Possess a sensitivity and aptitude for working with the elderly.
  • Possess skills to communicate effectively with those you supervise, residents, family members.
  • Must be able to prioritize and organize work effectively and efficiently.
