Gross Blended Rate: $60- $72 p/h

Key responsibilities
-
Perform comprehensive PT evaluations (strength, ROM, gait, balance, transfers, functional mobility) for adult/geriatric SNF residents.

Develop and implement individualized plans of care to improve mobility, independence, and safety.

Provide therapeutic exercise, gait/stair training, neuromuscular re‑education, and balance/coordination interventions.

Collaborate with an interdisciplinary team (OT, SLP, nursing, social work, physicians) on care and discharge planning.

Document evaluations, daily treatments, progress notes, and discharges accurately in the EMR.
Required qualifications (PT)

Active New Hampshire PT license (or eligibility before start).

Graduate of an accredited PT program.

Current BLS/CPR certification.

SNF or post‑acute experience preferred.

Key responsibilities

Document all treatment sessions and progress in the EMR.
