Home Health Occupational Therapist (OT)

Location: Sacramento

As a Home Health Occupational Therapist, your key responsibilities will include:

  • Delivering occupational therapy to patients who struggle with daily tasks due to illness or injury
  • Collaborating with a home health team, which may consist of an RN, LPN, CNA, HHA, PT, and SLP, to focus on the patient’s care plan and personal objectives
  • Evaluating patients, identifying deficits, setting therapy goals, and documenting progress
  • Guiding patients through exercises aimed at enhancing fine motor skills and coordination
  • Recommending adaptive equipment, such as grab bars and shower chairs, to provide additional support
  • Assessing fall risks and implementing strategies to enhance home safety
  • Educating patients and their families about the care plan, exercises, goals, and self-care techniques

Essential Qualifications for Home Health Occupational Therapists:

  • Graduation from an accredited Occupational Therapy Program and possession of an active OT license in California
  • At least 1 year of experience in occupational therapy, preferably in home healthcare settings
  • Current CPR certification [and any additional relevant certifications]
  • Familiarity with state and federal home health regulations
  • Strong clinical judgment, excellent interpersonal skills, resourcefulness, and compassion
