Job DescriptionCare Coordinator (Remote) - Contract to Hire
Schedule: Monday-Friday, 8:00 AM - 6:00 PM EST (40 hours/week)
Pay Rate: $20.00 - $21.00/hour
Start Date: ASAP or with 2 weeks' notice
Employment Type: 5-month contract-to-hire (through Medix) with strong conversion potential
Overview
A growing healthcare organization is seeking a Remote Care Coordinator to support a cardiology-focused patient population. This role is heavily focused on outbound patient outreach, enrollment, and care coordination, making it ideal for candidates who enjoy engaging with patients and working in a fast-paced, tech-enabled environment.
Strong preference for candidates located in EST or CST time zones.
Key Responsibilities
- Conduct outbound calls to engage and enroll eligible patients into care programs
- Guide patients through onboarding, consent, and enrollment processes
- Perform medical history intake and ensure accurate EMR documentation
- Support patient identification and intake workflows
- Conduct medication reconciliation (cardiology/heart failure preferred)
- Provide patient education, follow-up, and ongoing support
- Assist with appointment scheduling and care coordination
- Communicate updates effectively to clinical teams and providers
- Collaborate cross-functionally to ensure a seamless patient experience
Required Qualifications
- Experience with cardiology outreach or patient-facing outbound calling
- Strong technical skills (EMR systems, Microsoft Office)
- Reliable high-speed internet connection
- Excellent communication and interpersonal skills
- Detail-oriented and attentive
Preferred Qualifications
- Certified Medical Assistant (CMA) - preferred, not required
- Experience in virtual cardiology or heart failure programs
- Bilingual Spanish

IT Equipment Policy (Important)

Additional Information
- This is not an offer of employment
- Onboarding includes: background check, drug screening, and health screening
- You will be employed by Medix during the contract period
- Benefits eligibility begins after averaging 30 hours/week over 4 consecutive weeks
