Pharmacist
Department: Pharmacy • Status: Per-Diem • Location: South Miami, FL • FLSA: Non-Exempt
About the Role
Larkin Community Hospital serves one of South Florida’s most diverse and medically underserved communities. As a Pharmacist on our team, you will be at the center of direct patient care — working closely with physicians, nurses, and clinical staff to ensure safe, effective medication therapy across inpatient and outpatient settings.
This is a hands-on clinical role in an acute care hospital environment. You will manage complex medication regimens, support high-acuity units, contribute to formulary stewardship, and help lead a pharmacy technician team. Your expertise will directly impact patient outcomes for a population that depends on our hospital as a primary healthcare resource.
Minimum Qualifications
- Doctor of Pharmacy (PharmD) degree from an ACPE-accredited institution
- Active, unrestricted Florida pharmacist license in good standing (or eligibility to obtain prior to start date)
- Strong foundation in pharmacology, drug interactions, and pharmaceutical calculations
- Demonstrated ability to counsel patients clearly and collaborate effectively with interdisciplinary care teams
- Working knowledge of federal and Florida state pharmacy regulations, including DEA and Board of Pharmacy requirements
Preferred Qualifications
- 1+ years of experience in an acute care, inpatient, or health-system pharmacy setting
- Experience with IV admixture, sterile compounding, or clinical pharmacy services (e.g., anticoagulation, renal dosing)
- Proficiency with pharmacy information systems (e.g., Meditech, Cerner, or similar EHR/PIS platforms)
- Board certification (e.g., BCPS, BCACP) or completion of a PGY-1/PGY-2 residency program
- Certification in immunization delivery (APhA or equivalent)
- Bilingual (English/Spanish or English/Haitian Creole) strongly preferred given our patient population
- Training or certification in Medication Therapy Management (MTM)
Key Responsibilities
Clinical & Patient Care
- Verify, process, and dispense prescription medications accurately in accordance with legal, ethical, and hospital standards
- Provide clinical review of medication orders for appropriateness, dosing, interactions, and contraindications
- Counsel patients on medication use, expected effects, potential side effects, adherence strategies, and proper storage
- Collaborate with physicians, nurses, and clinical pharmacists to optimize individualized medication therapy plans
- Participate in clinical rounds or pharmacy consult services as assigned
Operations & Compliance
- Manage pharmacy inventory including procurement, stocking, controlled substance reconciliation, and expiration date monitoring
- Maintain full compliance with federal, state, and hospital pharmacy regulations and accreditation standards (TJC, CMS)
- Document all pharmacy activities, patient counseling notes, and clinical interventions accurately in the EHR
- Contribute to drug utilization reviews, adverse drug event reporting, and pharmacy quality improvement initiatives
Leadership & Education
- Supervise, train, and mentor pharmacy technicians and support staff to ensure quality service and regulatory compliance
- Support onboarding and competency assessment of new pharmacy team members
- Participate in community health promotion events and patient education initiatives aligned with Larkin’s mission
- Assist with the precepting of pharmacy students or residents as applicable
Work Environment & Schedule
- Acute care hospital setting with potential rotation across inpatient areas
- Scheduled shifts may include evenings, weekends, and holidays on a rotating basis
- On-call responsibilities may be required based on departmental needs
- Fast-paced environment requiring the ability to prioritize multiple competing demands
