Our service area includes: Arlington, Portage, Rio, Wyocena, Dane, Deforest, Doylestown, East/North Bristol, Lodi, Morrisonville, Windsor, Briggsville, Cambria, Dalton, Endeavor, Fox Lake, Kingston, Lewiston, Marquette, Montello, Oxford, Packwaukee, Pardeeville, Plainville, Westfield, Lake Mills, Lowell, Reesville, Waterloo, Beaver Dam, and Columbus.
- Work Schedule: Monday to Friday, 8:00 AM - 4:30 PM (On-call one weekday approximately every two weeks; no weekend shifts)
- Compensation: $0.70 per mile reimbursed in addition to bi-weekly pay

Job Overview:
The Home Health Nurse will coordinate, facilitate, and implement an interdisciplinary care plan by assessing and evaluating patient conditions and the effectiveness of care under the guidance of the attending physician. This role involves traveling to patient homes, Community-Based Residential Facilities (CBRF), assisted living facilities, hospitals, and other locations to meet with patients and their families/caregivers, assess care needs, and ensure compliance with home care and hospice regulations.
Key Responsibilities and Qualifications:
PRIMARY DUTIES
- Evaluate and coordinate discharge planning needs with the healthcare team.
- Conduct point-of-care testing as per established policies.
- Collaborate with physicians regarding care plans and patient status updates, documenting all communications in compliance with regulations.
- Coordinate with multidisciplinary team members and external agencies to achieve desired patient care outcomes and education, ensuring thorough documentation of care plans.
- Complete admission paperwork and verify insurance details, conducting comprehensive medical assessments in a timely manner.
- Deliver skilled nursing care as outlined in the care plan, including vital signs monitoring, wound care, IV therapy, ostomy care, blood draws, injections, and overall patient assessments.
- Document visit notes and treatments accurately, adhering to physician orders and ensuring timely completion of required visits.
- Work collaboratively with patients, families, and the healthcare team to create individualized care plans, responding to inquiries and addressing concerns promptly.
- Maintain a constant state of alertness and safety.
- Perform additional duties as assigned.
EDUCATION
- Graduate from an accredited nursing program or equivalent education for licensure.
EXPERIENCE
- No prior experience is necessary.
ADDITIONAL REQUIREMENTS
- Proof of motor vehicle liability insurance that meets state minimum requirements upon hire and upon request.
PHYSICAL DEMANDS
- Regular verbal communication to convey information.
- Frequent standing and walking.
- Occasional lifting/carrying and pushing/pulling of objects weighing 0-25 lbs.
- Frequent sitting, standing, walking, reaching, and repetitive movements.
- Regular use of vision for near and far distances, as well as color differentiation.
- Frequent use of hearing for communication and to detect alarms or machinery issues.
- Regular driving and data entry.
- Occasional lifting/moving of patients and objects weighing 25-50 lbs.
- Occasional bending, stooping, kneeling, and twisting.
- Rare crawling and running.
REQUIRED LICENSES AND CERTIFICATIONS
For Illinois:
- Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA)
- Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Valid Driver's License and Motor Vehicle Report (MVR)
For Missouri:
- Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA)
- Registered Nurse (RN) - Issued by Compact State or Missouri Division of Professional Registration
- Valid Driver's License and Motor Vehicle Report (MVR)
For Oklahoma:
- Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA)
- Registered Nurse (RN) - Issued by Compact State or Oklahoma Board of Nursing (OBN)
- Valid Driver's License and Motor Vehicle Report (MVR)
For Wisconsin:
