Position Details:
- Schedule: Full-time
- Location: Scottsdale, AZ and surrounding areas
- Experience: Minimum 1 year of skilled home health experience required
- EMR: Kinnser (WellSky) experience preferred

Responsibilities:
- Provide skilled nursing visits in the home, including assessments, medication administration, and care coordination
- Develop and update individualized care plans in collaboration with the care team
- Educate patients and caregivers on disease management and home care techniques
- Accurately document all care using our EMR (Kinnser/WellSky)
- Communicate effectively with physicians, team members, and families to ensure quality outcomes
Requirements:
- Active RN license in the state of Arizona
- 1 year of recent skilled home health experience using Oasis required to be considered
- Strong clinical and organizational skills
- Ability to work independently and manage a patient caseload effectively
- EMR experience a plus (Kinnser/WellSky preferred)
