• Weekends: Every other weekend\n
• Holidays: Rotating schedule\n
• On-Call: 1-2 times/week and every other weekend (triage calls)\n
• EMR: Epic\n
• Mileage Reimbursement: $0.70 per mile\n
• Estimated Weekly Salary: $2100-$2260\n

\n\nRN Qualifications\n

\n
• Active Registered Nurse (RN) license in Wisconsin\n
• BLS certification (American Heart Association)\n
• Minimum 1 year of nursing experience; homecare or case management experience preferred\n
• Strong clinical assessment and critical thinking skills\n
• Valid driver's license and reliable transportation\n
• Ability to work independently and within an interdisciplinary healthcare team\n
• Excellent communication and organizational skills\n

\n\nRN Homecare Responsibilities\n

• Coordinate and manage patient care in a homecare setting, ensuring high-quality, patient-centered outcomes\n
• Conduct comprehensive nursing assessments and develop individualized care plans\n
• Educate patients and families on disease management, medications, and home safety\n
• Collaborate with physicians, therapists, and other healthcare professionals for continuity of care\n
• Document patient interactions and care activities accurately in Epic EMR\n
• Participate in on-call rotation, triaging patient calls and determining appropriate interventions\n
• Adhere to all regulatory and compliance standards for Wisconsin nursing jobs\n
