Physical Therapist Position! We will Train!
Home Health Physical Therapist needed to evaluate patients, develop individualized treatment plans, and provide direct patient care to improve mobility, balance, and strength within the patient's home. You will also focus on patient and family education, fall prevention, documenting progress, and collaborating with the healthcare team.
Willing to train, if you are a recent graduate with a Physical Therapist License
Patient care and treatment
- Assess patient needs: Evaluate patients' physical capabilities and limitations in their home environment to diagnose and treat conditions.
- Develop treatment plans: Create and implement individualized plans using exercises, manual therapy, and other physical means like heat, massage, or electrical stimulation.
- Provide direct treatment: Guide patients through exercises to relieve pain, restore function, and improve strength and range of motion.
- Fall prevention: Assess home environments for hazards and teach patients how to use assistive devices like walkers or canes safely.
- Use equipment: Utilize therapeutic equipment such as whirlpools, heat/ice packs, and strength/mobility training tools.
Documentation and administration
- Maintain records: Keep detailed and timely records of patient progress, treatment goals, and any changes in condition.
- Report to physicians: Observe and report patient reactions to treatment and their overall condition to the physician.
- Adapt plans: Modify treatment plans based on the patient's status and progress.
Education and collaboration
- Educate patients and families: Instruct patients and their families on in-home treatment options, proper techniques for mobility, and safety measures.
- Collaborate with the healthcare team: Work with physicians, case managers, and other medical professionals to coordinate patient care.

· We follow a pay per visit model-Visit rates:
Start of Care-$172.50-$197.50
Evaluation -$91-$104
Home Visit-$69-$79
Hourly $45-$50 (includes driving time)
· Benefits (Medical, Dental, Vision) company contributes $475/month to each employee to apply towards their healthcare plans (several plans to choose from)
· 401K contribution is currently a10% match of whatever the employee contributes annually.
· PTO- 2 weeks the first year and then 3 weeks after 2 yrs of employment
