Job Description

Essential Functions/Areas of Accountability

· Use the DC Medicaid DC Care Connect portal for Care coordination with patients, case managers, HHAs, RNs, and caregivers

· Tracking of recertification completed by Case Managers in DCCC, coordinating care regarding recertification as needed, and generating a report from HHC using authorization due tracking.

· Completing ePOF in DCCC to Telligen and following up with the physician's office and/or family member for signature

· Participate in developing POC/485 with the clinicians, including PCSP

· Update Prior Authorizations (PAs)

· Assist with payroll and review timesheets vs. EVV records from Sandata

· Submit and manage documents via DC Care Connect

· Submit and follow up on EPPR after submission

· Complete and track EPOFs, InterRAI, and PCSP submissions

· Coordinate with Comagine and Telligen for authorizations

· Report incident SRI and RI within 24 hours and follow up appropriately in DC Care Connect to close out

· Support general administrative and office duties

· Reviews client referral information and responds suitably to requests and inquiries.

Run an employee required event report and ensure all employees have current work authorizations, renewal of licenses and certifications as required by the Federal and State employment law
