Job DescriptionOpportunity Overview

We are looking for a dedicated and skilled Nurse Practitioner (NP) to assist our healthcare providers through a combination of in-basket management, virtual clinical support, and direct patient care. This position is perfect for an NP who thrives in a collaborative environment, enjoys a dynamic clinical setting, and is adept at managing both virtual and in-clinic workflows.

Job Responsibilities

  • Clinical Support & Patient Care:
    • Deliver in-person patient care, which includes assessments, diagnoses, treatment planning, and follow-up.
    • Conduct virtual consultations for triage, follow-ups, chronic condition management, and patient education.
    • Facilitate patient access by addressing urgent issues and clearly communicating care plans.
  • In-Basket Management:
    • Manage provider in-basket items such as patient messages, medication refill requests, lab/test results, and care coordination tasks.
    • Respond to or appropriately route items to ensure timely and accurate resolutions.
    • Escalate clinical concerns to supervising physicians as necessary.
  • Care Coordination & Communication:
    • Work closely with physicians, RNs, MAs, and administrative staff to ensure seamless patient care.
    • Maintain accurate and timely documentation in the Electronic Health Record (EHR).
    • Engage in team huddles, quality initiatives, and workflow improvement projects.

Requirements and Qualifications

  • Education:
    • Graduation from an accredited nursing program.
    • Master's degree in Nursing.
  • Certification & Licensure:
    • California Nurse Practitioner license from the California Department of Consumer Affairs.
    • Nurse Practitioner Furnishing (NP-F) certification.
    • California Registered Nurse (RN) license.
    • Controlled Substance Registered Certification (DEA) - not required for non-patient care roles.
    • Basic Life Support (BLS) certification as required by the department.
  • Typical Experience:
    • Current, unrestricted Nurse Practitioner license in the state of practice.
    • National board certification (FNP, AGNP, ANP, or equivalent specialty).
    • A minimum of 5 years of clinical NP experience.
    • Experience in telehealth or virtual care is a plus.
    • Strong communication skills and ability to manage a high volume of electronic in-basket tasks.
    • Capability to prioritize, multitask, and work independently.
  • Skills and Knowledge:
    • Proficient understanding of advanced nursing theories, medical terminology, anatomy, and physiology.
    • Familiarity with healthcare industry standards, safety policies, best practices in patient care, and regulatory changes.
    • Effective written and verbal communication skills to convey sensitive information clearly to diverse audiences.
    • Basic proficiency in computer applications, including Microsoft Office Suite and Electronic Health Records.
    • Able to prioritize and delegate tasks while adhering to established policies and procedures.
    • Ability to work independently and collaboratively within a team, managing multiple tasks in a dynamic environment.
    • Capable of identifying, evaluating, and resolving standard problems by selecting appropriate solutions.
    • Commitment to maintaining the privacy of each patient’s protected health information (PHI).
    • Ability to build collaborative relationships with peers and other healthcare professionals to meet departmental and organizational goals.
