Job DescriptionJob Opportunity: Physical Therapist

Our organization is in search of a dedicated and empathetic Physical Therapist to become a part of our Healthcare & Medical Services team. This position focuses on delivering evidence-based rehabilitation services primarily to senior and post-acute patients within a nurturing, family-like atmosphere.

Key Responsibilities

  • Conduct thorough physical therapy assessments.
  • Design personalized treatment plans tailored to individual patient needs.
  • Implement skilled interventions aimed at helping residents regain functionality, enhance mobility, alleviate pain, and promote independence.
  • Collaborate closely with a multidisciplinary team, including nurses, physicians, occupational and speech therapists, and social workers, to develop patient-centered care plans that emphasize safety, dignity, and quality outcomes.
  • Provide therapeutic exercises, balance training, gait and transfer training, manual therapy, neuromuscular re-education, and functional mobility activities aligned with each patient’s goals.
  • Educate patients and their families on home exercise routines, fall prevention strategies, safe use of assistive devices, and methods for maintaining functionality post-discharge.
  • Ensure accurate and timely documentation in the electronic health record, including evaluations, progress notes, and discharge summaries, to support regulatory compliance and optimal reimbursement.
  • Engage in team meetings, case conferences, and continuous education to remain updated on best practices in geriatric and rehabilitation therapy.

Requirements and Qualifications

  • Valid Physical Therapist license.
  • Strong commitment to patient-centered care.
  • Ability to work collaboratively in a team-oriented environment.
  • Excellent communication and interpersonal skills.
