Job Title: Patient Care Coordinator
Job Summary:
We are seeking a dedicated and compassionate Patient Care Coordinator to join our home-based primary care practice specializing in geriatrics and palliative care. The ideal candidate will play a crucial role in ensuring that our patients receive high-quality care and support in a home setting. This position requires strong organizational skills, excellent communication abilities, and a commitment to enhancing the patient experience.
Key Responsibilities:
- Coordinate patient related cares, including scheduling appointments, managing referrals, and ensuring timely follow-up.
- Serve as the primary point of contact for ACO criteria, addressing inquiries and providing support throughout the care process.
- Collaborate with healthcare providers to develop and implement individualized care plans that meet the unique needs of geriatric and palliative patients.
- Maintain accurate and up-to-date patient records, ensuring compliance with healthcare regulations and standards.
- Educate patients and families about available resources, services, and care options to promote informed decision-making.
- Monitor patient progress reports dashboard and communicate any changes in condition to the healthcare team.
- Assist in the development and implementation of quality improvement initiatives to enhance patient care and satisfaction.
- Assist QA Provider with chart audits, ACO and regulatory compliance.
Requirements:
Qualifications:
- Previous experience in patient care coordination, particularly in geriatrics or palliative care, is highly desirable.
- Strong interpersonal and communication skills, with the ability to build rapport with patients and families.
- Excellent organizational skills and attention to detail.
- Proficient in using electronic health record (EHR) systems and other healthcare software.
- Proficient in using Microsoft Office platform and other electronic devices, iPad, cell phone, etc...
- Ability to work independently and as part of a collaborative team.
- Compassionate and empathetic approach to patient care.
- Reliable transportation. This position will require visiting patients face-to-face from time to time.
