Job Overview

As a Home Health Registered Nurse (RN), you will work collaboratively with your team to develop treatment plans tailored to each patient. You will also have the opportunity to creatively utilize available resources in their homes, educate patients and their families, and adapt care strategies to meet the changing needs of your patients.

Service Areas

This role covers the following counties in Illinois: Clinton, Washington, Perry, Marion, Jefferson, Franklin, Wayne, Hamilton, and Edwards.

Compensation

Salary Range: $32.77 - $52.43 per hour

The stated pay range reflects what we reasonably anticipate offering at the time of posting, but actual pay may vary in accordance with legal guidelines.

Key Responsibilities

  • Evaluate and coordinate discharge planning needs in collaboration with the healthcare team.
  • Conduct point-of-care testing as per established protocols.
  • Engage with physicians regarding care plans and any changes in patient status, providing regular updates and documentation in compliance with regulations.
  • Work with multidisciplinary team members and external agencies to ensure effective patient care and education throughout the discharge process or end-of-life care, maintaining thorough documentation.
  • Complete admission forms and verify insurance details, ensuring timely completion of comprehensive medical assessments using clinical software.
  • Deliver skilled nursing care as outlined in the care plan, including vital sign monitoring, wound care, IV therapy, ostomy care, blood draws, injections, and overall patient assessments.
  • Document visit notes, treatments, and care accurately and promptly, adhering to physician orders and regulatory requirements.
  • Collaborate with patients, their families, and the healthcare team to create a personalized care plan that addresses individual needs, responding to inquiries and following up on concerns.
  • Maintain a vigilant and safe working environment.
  • Perform additional duties as assigned.

Qualifications

  • Education: Graduate from an accredited nursing program or equivalent educational background for licensing.
  • Experience: No prior experience is necessary.
  • Additional Requirements: Proof of motor vehicle liability insurance that meets state minimums upon hire and upon request.

Physical Demands

  • Regularly communicate verbally to share information.
  • Frequent standing and walking are required.
  • Occasional lifting, carrying, and pushing/pulling of objects weighing between 0-50 lbs.
  • Regularly engage in activities such as sitting, reaching, and repetitive movements of the arms and legs.
  • Consistent use of vision for tasks at varying distances and to distinguish colors.
  • Regularly use hearing to receive oral communication and detect alarms or machinery issues.
  • Frequent driving and data entry tasks.

Licensing and Certifications

Applicants must hold the following certifications based on their state of work location:

  • Illinois: Basic Life Support Healthcare Provider (BLS HCP) from the American Heart Association, Registered Nurse (RN) license from the Illinois Department of Financial and Professional Regulation, and a valid driver’s license.
  • Missouri: BLS HCP, RN license from a Compact State or Missouri Division of Professional Registration, and a valid driver’s license.
  • Oklahoma: BLS HCP, RN license from a Compact State or Oklahoma Board of Nursing, and a valid driver’s license.
  • Wisconsin: BLS HCP, RN license from a Compact State or Wisconsin Department of Safety and Professional Services, and a valid driver’s license.
