Registered Nurse (RN) – Home Health
Schedule: Monday–Friday, 8:00 AM–5:00 PM
Some flexibility for weekend coverage is preferred, including approximately 1 weekend per month and 1 on-call night per week.
License Requirement:
Florida RN license required; compact license accepted
Mileage Reimbursement:
$0.43 per mile
Coverage Area:
Escambia, Santa Rosa, Okaloosa, and Walton counties
Required Skills:
OASIS and HCHB experience are required. Additional preferred skills include wound care, PICC/IV care, lab collection, drain care, ostomy care, and post-surgical care experience.
Position Overview:
The Registered Nurse (RN) is responsible for delivering skilled home health care while completing comprehensive assessments and coordinating care for high-risk patients. This role works closely with physicians, social workers, discharge planners, and other healthcare professionals to develop and implement individualized care plans that support positive patient outcomes.
Key Responsibilities:
- Perform comprehensive assessments of high-risk patients in the home setting
- Develop, implement, and continuously reassess individualized care plans
- Collaborate with primary care providers and interdisciplinary teams to coordinate patient care
- Facilitate smooth care transitions and connect patients with appropriate resources and support services
- Identify opportunities for health promotion, illness prevention, and improved patient outcomes
- Prevent adverse patient events when possible and intervene promptly when needed
- Maintain accurate and timely documentation of clinical, functional, and fiscal outcomes
- Advocate for patients while protecting privacy and confidentiality
- Provide skilled nursing care including wound care, PICC/IV care, lab collection, drain care, and ostomy care as needed
- Complete OASIS documentation accurately and utilize HomeCare HomeBase/PointCare effectively
Qualifications:
- Associate’s or Bachelor’s degree in Nursing or related field
- Current, unrestricted RN license
- Strong clinical judgment and ability to analyze complex medical information
- Proficiency in HomeCare HomeBase/PointCare and OASIS documentation required
