Salary Range: $58,656.00 - $66,456.00 annually

ACT is a program that functions as a “hospital without walls”. To this extent the role of the ACT team is to provide services and supports to individuals most affected by mental illness, who without this service would be involved in institutional care. The role of ACT and the Case Manager is to provide services that will stabilize an individual’s condition, aid and support to prevent hospitalization and to focus on the recovery process. As a member of the ACT team, there is shared responsibility for providing the services and supports necessary to assist everyone as they work towards their goal of recovery.

Main Duties and Responsibilities:
- Responsible for working as a team, in concert with everyone to develop an individualized plan of services and supports (Individual Treatment Plan) that will enable each person to live as independently in the community as possible. Services and supports might include activities such as assisting with skill development in the following areas: budgeting, housing, transportation, hygiene appointments, relationship building, symptom management, medication administration and obtaining or maintaining benefits.
- Maintains awareness of everyone’s medication adherence plan and provides services to assist individuals in carrying out the plan; evaluates adequacy of plan, reports problems and recommends modifications as needed.
- To the extent requested by the individual, provide collaboration and education to significant others and family members who are interested in assisting the client in their recovery process.
- Advocate on behalf of everyone to assure the protection of client rights and privacy, as well as assure that individuals understand the complaint and grievance procedures.
- Treat all individuals with dignity and respect, providing services that are respectful of age, gender, and culture.
- Assure that all changes in the individual’s status or needs are communicated promptly to the team to enhance continuity of care.
- Assure that services are provided as developed by the individual and team as documented in the Individual Treatment Plan.
- In case of illness or crisis, provide coverage for team members to assure and maintain continuity of care for each person served.
- Participate in all scheduled staff meetings and trainings, unless excused by your immediate supervisor.
- Act in a manner to assure individual’s confidentiality and privacy that complies with MHR policies and procedures.
- Assure that all MHR documentation is completed on time and within MHR policies and procedures.
- Assure all mandated civil commitment procedures and process are followed and information is submitted as required according to timelines required by law.
- Assure that MHR productivity standards (i.e. assertive outreach, case load, face-to-face contact, contact frequency, on-call, shift leader, PM, etc.) are met, as outlined by the Team Leader
- Chart maintenance – ensure chart information is up to date (i.e., ROIs, profile information, status changes, contacts, etc.).
Minimum Education and Experience:
- Baccalaureate degree in social work, psychology or human services related field.
- Two years case management experience of which at least one year is experience in community based mental health with adults recovering from mental illness OR combination of equivalent experience and training.
