Position Summary:
We are hiring a Wound Care focused Physician Assistant (PA-C) to provide on-site rounding, advanced wound management, and comprehensive primary care. This role is built for clinicians who want to expand their clinical scope, manage complex cases, and play a key role in driving patient outcomes. You will serve as a trusted clinical resource, working closely with interdisciplinary teams to elevate care delivery across your assigned communities.
Responsibilities:
• Deliver on-site rounding, primary care, and advanced wound care services.
• Manage pressure injuries, diabetic ulcers, surgical wounds, and skin integrity issues.
• Lead chronic disease management and preventative care initiatives.
• Collaborate with physicians, nurses, therapists, and facility leadership.
• Reduce hospitalizations through early intervention and ongoing monitoring.
• Communicate with families, caregivers, and care teams.
• Complete documentation, medication management, and lab coordination.
Qualifications:
• Active Minnesota Physician Assistant (PA-C) license in good standing.
• Experience in wound care, geriatrics, primary care, or post-acute care preferred.
• Strong clinical decision-making and ability to manage complex patient populations.
• Valid driver’s license and reliable transportation.
• Commitment to high-quality, patient-centered senior care.

• Air Force: 42G3 – Physician Assistant.
• Marines: Physician Assistant (230X).
