Schedule: Monday–Friday, 9:00 AM – 5:00 PM
Shift: Day Shift
Hours: 35–40 Hours per Week
Compensation: $62–$77 per Hour (Based on Experience)
Position Overview
We are seeking an experienced Licensed Master Social Worker (LMSW) to support an inpatient medicine and surgery team within a hospital setting. The LMSW will provide psychosocial assessments, supportive counseling, care coordination, and discharge planning services to patients and families throughout the hospitalization process.
The ideal candidate will demonstrate strong clinical judgment, effective communication skills, and the ability to collaborate with multidisciplinary healthcare teams to ensure safe patient transitions and positive outcomes.
Responsibilities
- Conduct comprehensive psychosocial assessments for hospitalized patients.
- Provide supportive counseling and short-term therapeutic interventions.
- Develop and coordinate individualized discharge plans.
- Facilitate referrals to community resources, rehabilitation programs, and support services.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals.
- Document assessments, interventions, and treatment plans accurately within the medical record.
- Assist patients and families in coping with illness, hospitalization, and recovery.
- Support continuity of care through effective discharge coordination and follow-up planning.
- Participate in interdisciplinary rounds and patient care conferences.
- Maintain compliance with regulatory standards, hospital policies, and social work best practices.
Required QualificationsEducation
- Master’s Degree in Social Work (MSW) from an accredited program.
Licensure & Certifications
- Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED).
- Primary Source Verification of LMSW license required.
Experience
- Minimum one (1) year of LMSW experience required.
- Previous inpatient hospital experience preferred.
