Position Overview:

As a Home Health Registered Nurse (RN), you will work collaboratively with your team to develop treatment plans for patients. You will also have the opportunity to creatively utilize resources available in their homes, educate patients and their families, and adapt care strategies to meet the changing needs of your patients.

Service Areas:

Clinton, Washington, Perry, Marion, Jefferson, Franklin, Wayne, Hamilton, and Edwards Counties in Illinois.

Compensation Range:

$32.77 - $52.43

Job Summary:

This role involves coordinating, facilitating, and executing an interdisciplinary care plan through the assessment and evaluation of patient conditions and the effectiveness of care, all under the guidance of the attending physician. You will travel to patient homes, Community-Based Residential Facilities (CBRF), assisted living centers, hospitals, and other locations to meet with patients and their families, assess care needs, and ensure compliance with home care and hospice regulations.

Key Responsibilities:

  • Evaluate and coordinate discharge planning needs with the healthcare team.
  • Conduct point-of-care testing as per established policies.
  • Engage with physicians regarding care plans and patient status updates, documenting all communications in compliance with regulations.
  • Collaborate with multidisciplinary team members and external agencies to ensure effective patient care and education through discharge or end-of-life processes, maintaining thorough documentation.
  • Complete admission forms and verify insurance details, ensuring timely completion of comprehensive medical assessments.
  • Deliver skilled nursing care as outlined in the care plan, including vital sign checks, wound care, IV therapy, ostomy care, blood draws, injections, and overall patient assessments.
  • Document visit notes, treatments, and care provided, adhering to physician orders and ensuring timely completion of required visits.
  • Work closely with patients, their families, and the healthcare team to create a tailored care plan that addresses individual patient needs, responding to inquiries and concerns promptly.
  • Maintain a state of alertness and safety at all times.
  • Perform additional duties as assigned.

Education Requirements:

  • Graduate from an accredited nursing program or equivalent educational background for licensing.

Experience:

  • No prior experience is necessary.

Additional Requirements:

  • Must provide proof of motor vehicle liability insurance that meets state minimum requirements upon hire and upon request.

Physical Demands:

  • Regularly use speech for effective communication.
  • Frequent standing and walking required.
  • Occasional lifting, carrying, and pushing/pulling of objects weighing 0-25 lbs.
  • Regular sitting, standing, walking, reaching, and repetitive movements of feet/legs and hands/arms.
  • Frequent visual acuity for near (20 inches or less) and far (20 feet or more) distances, including color differentiation.
  • Regularly use smell to identify odors.
  • Consistent hearing ability to receive verbal communication and detect alarms or machinery issues.
  • Frequent driving required.
  • Regular keyboard use and data entry.
  • Occasional lifting of objects weighing 25-50 lbs.
  • Occasional patient lifting/moving.
  • Occasional bending, stooping, kneeling, squatting, twisting, gripping, and repetitive movements.
  • Rarely required to crawl or run.

Required Licenses and Certifications:

For Illinois:

  • Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA)
  • Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
  • Valid Driver's License and Motor Vehicle Report (MVR)

For Missouri:

  • Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA)
  • Registered Nurse (RN) licensed by Compact State or Missouri Division of Professional Registration
  • Valid Driver's License and Motor Vehicle Report (MVR)

For Oklahoma:

  • Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA)
  • Registered Nurse (RN) licensed by Compact State or Oklahoma Board of Nursing
  • Valid Driver's License and Motor Vehicle Report (MVR)

For Wisconsin:

  • Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA)
  • Registered Nurse (RN) licensed by Compact State or Wisconsin Department of Safety and Professional Services
  • Valid Driver's License and Motor Vehicle Report (MVR)
