In 2024 Fraser became a Certified Community Behavioral Health Clinics (CCBHC). Allowing Fraser to meet the increasing need for autism, mental health, and disability services in our community by providing our clients with faster access to treatment, integrating services with primary care, healthcare providers, and community agencies to ensure our clients have the best opportunity to succeed.
As a member of Fraser’s CCBHC, the Registered Nurse is responsible for proactively supporting medical, social and behavioral health conditions for clients from intake through treatment. This individual works collaboratively across departments to ensure continuous quality improvement, addressing client experience and support of physical health measures.
Requirements:
- Current Registered Nurse Licensure with the State of Minnesota.
- Public Health Nursing Certification with the State of Minnesota.
- BS or BA in Nursing from an accredited college or university.
RN Responsibilities:
- Build rapport and establish effective working relationships with clients and families.
- Coordinate client screening and outcomes with members of the individual’s healthcare team.
- Participate in the assessment, planning and development of integrated physical health and mental health services.
- Participate in the training and consultation of internal and external partners.
- Participate in Fraser Team Care to support client whole person health.
- Complete thorough and accurate documentation of client encounters, maintain clinical files and up to date records.
- Provide Primary Care Screening and Monitoring including: collecting CCBHC measures, assessing primary care coordination and unmet health risks.
- Track client medications, lab results and use of routine and preventive primary care, dental care and well child PCP visits.
- Support the integration of physical health into comprehensive care planning.
- Collaborate with team members across programs; provide consultation as needed.
- Provide input in the development of policies, procedures and standards of practice for Health and Wellness at Fraser.
Administrative Responsibilities:
- Complete documentation in a timely manner as outlined in the regulations and in accordance with Fraser's policies and procedures. Documents care coordination activities in EMR system.
- Maintain all records in accordance with HIPAA requirements.
- Accurately document services delivered and submit all billing to Accounting department with monthly requirements.
Professional Growth & Development
- Seek out training opportunities as necessary to remain current with best
- Complete required continuing education and training in compliance with
- Promote teamwork and collaboration within the Fraser Behavioral and Mental Health program teams and throughout Fraser.
Location, Pay & Schedule:
- Location: Bloomington, MN.
- Pay Range for 32 hours per week: $68,800 - $72,000 at 0.8 FTE
- Schedule: 4 days a week, Monday-Thursday, standard business hours, ability to travel as needed and work in office one day a week
