Location: Fairport, NY
Employment: Full-Time / Part-Time / Per Diem
Shifts: Days / Evenings / Overnights
Pay: $18.65–$19.65 per hour

We are seeking dependable, compassionate, and resident-focused Home Health Aides (HHA) / Personal Care Aides (PCA) to join our assisted living team. In this role, you will provide hands-on care and supportive services based on each resident’s service plan, physician orders, facility policies, and applicable New York State requirements.

Position Summary

The HHA/PCA provides personal care, assistance with daily living, medication support, meal assistance, companionship, and other services designed to help residents maintain the highest level of independence appropriate to their abilities.

The ideal candidate is compassionate, dependable, observant, and comfortable working as part of a healthcare team.

Essential ResponsibilitiesPersonal Care & Resident Support

  • Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, and other personal care needs identified in the resident’s service plan.

  • Encourage residents to maintain the highest level of independence appropriate to their abilities.

  • Provide reminders and supportive assistance with daily routines.

  • Assist residents with mobility, transfers, positioning, and ambulation as needed.

  • Provide companionship and emotional support.

  • Observe residents and promptly report changes in condition, behavior, functioning, or other concerns to the appropriate supervisor.

  • Follow each resident’s current service plan and established care procedures.

  • Protect resident privacy, dignity, confidentiality, and rights at all times.

Medication Assistance & Healthcare Coordination

  • Provide medication assistance within the scope of training, authorization, facility policy, physician orders, and applicable regulations.

  • Follow current medication orders and promptly report discrepancies, missing medications, refusals, or other medication concerns.

  • Assist with obtaining medication refills and new prescriptions according to facility procedures.

  • Communicate medication changes and related concerns to the appropriate supervisor.

  • Assist with coordinating and scheduling physician and other healthcare appointments as directed.

  • Communicate with physicians, medical offices, pharmacies, and other healthcare providers as authorized and directed.

  • Report changes in a resident’s condition to the appropriate healthcare professional and supervisor.

  • Receive, review, communicate, and follow applicable physician orders and healthcare-related instructions within the employee’s scope of responsibility.

  • Complete required medication and healthcare documentation accurately and in a timely manner.

  • Follow up on outstanding orders, prescriptions, appointments, and healthcare needs as assigned.

Meal Support

  • Assist with meal setup and serving.

  • Support residents with individual dining needs identified in their service plan.

  • Monitor and report concerns regarding nutrition, hydration, or changes in eating patterns.

  • Follow prescribed diets and dietary instructions as applicable.

Documentation & Communication

  • Complete all required documentation accurately, timely, and according to facility procedures.

  • Maintain required 24-hour communication logs, progress notes, medication-related documentation, and other resident records.

  • Document changes in condition, incidents, appointments, healthcare communications, and follow-up actions.

  • Communicate relevant resident information during shift-to-shift reporting.

  • Maintain strict confidentiality of resident information.

  • Promptly report incidents, accidents, changes in condition, and other concerns through the appropriate chain of command.

  • Communicate effectively with nursing staff, supervisors, residents, families, and other members of the care team.

Light Housekeeping & Daily Living Support

  • Assist with light housekeeping, laundry, meal preparation, and other daily living tasks as appropriate.

  • Help maintain a clean, safe, and comfortable environment for residents.

  • Follow all facility safety, infection-control, and emergency procedures.

Qualifications & Requirements

  • Current HHA or PCA certification required.

  • Current First Aid certification required.

  • High school diploma or equivalent preferred.

  • Previous experience working with older adults or in assisted living, home care, or a healthcare setting preferred.

  • Ability to follow resident service plans, physician orders, facility policies, and applicable regulations.

  • Strong communication, observation, documentation, and organizational skills.

  • Dependable, professional, compassionate, and able to work effectively as part of a team.

  • Ability to recognize and promptly report changes in a resident’s condition.

  • Ability to maintain resident confidentiality and respect resident rights.

  • Ability to safely lift up to 50 pounds and perform the physical requirements of the position.

  • Familiarity with electronic medical records (EMRs) preferred.

We are currently seeking team members for:
