Because of the variety of patients we see, you will be given the constant opportunity for experience and patient engagement. This means you should have the ability and willingness to:
- Serves as a preceptor and provides individual and group mentorship for unlicensed Social Workers and social worker interns in accordance with regulations of relevant State of California licensing boards.
- Collaborate with multi-disciplinary team in the coordination of care by evaluating, assessing and monitoring the patient care plan.
- May provide functional direction to unlicensed social work staff and social work interns.
- Participates in the coordination of transfers to alternative level of care as appropriate, including acute psychiatric facilities.
- Provide comprehensive assessments and referrals, crisis intervention and care management.
- Collaborate with multi-disciplinary team and other internal and external personnel to promote an awareness of environmental and psychosocial needs and to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community.
- Have excellent communication and problem-solving skills.
- Participates in interdisciplinary planning, case conferences, grant requirements, researches and/or quality improvement projects as needed.
Qualifications
Educationo Master's Degree in Social Work (MSW) requiredExperience• 2 years of experience in psychosocial and therapeutic assessments and interventions requiredLicenses and Certifications• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required• LCSW - State of California Licensed Clinical Social Worker required
